Recognizing that every person is unique,
Beechwood's program is based on each 
individual's goals, hopes and needs. This type
of personalized approach fosters a social relationship among the individuals served and their staff, which in turn creates a community of supports that are vital to emotional well-being.
Following admission, each person served participates in a comprehensive 30-day evaluation conducted by our interdisciplinary team. The results of this evaluation are shared with the individual and, when appropriate, their family and funding representative.
A Rehabilitation Service Plan is developed focusing on the goals and aspirations of the
individual served in concert with the team,
family and funding representative. The plan focuses on functional areas identified as barriers to successful re-entry into the community.
The status of each individual is reviewed by the assigned team every other week. Formal team meetings take place on a regular basis, ranging from every month to no more than every six months, from the time of admission through discharge. From the first day of admission, our planning promotes advancement toward the highest possible functional outcome.
